Visit our website and download the official application form directly from the “Apply & Enroll” page.

Complete and Sign the Form
Carefully fill out all required fields in the application form, ensuring accuracy. Don’t forget to paste your recent photo and provide your signature at the end of the form.

Scan the Signed Application
Use a scanner or a mobile scanning app to create a clear digital copy of the completed and signed application form.

Email the Scanned Form
Send the scanned application as an email attachment to admissions@iihcsm.com. Make sure to include any additional documents requested in the application instructions.

Await Our Response
